When the breast is droopy or saggy (ptotic) a simple augmentation may not do. In such cases a mastopexy or breast lift is need.
Some patients, especially those with relatively small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions.
One such procedure is the "doughnut (or concentric) mastopexy," in which circular incisions are made around the areola, and a doughnut-shaped area of skin is removed.
The main difference between Dr. Mendieta's techniques and those of other surgeons is that in order to achieve the best and the most natural looking results, Dr. Mendieta corrects the sagging breast gland first by lifting it and restoring it to its natural position. Once a nice breast shape is attained, an implant is then inserted along with your breast lift. The implant is placed in a pocket directly under the breast tissue, or deeper, under the muscle of the chest wall.
Dr. Mendieta's technique not only lifts the areola but also works on reshaping and lifting the gland itself, placing the breast tissue at a higher position. This gives the breast lift, a more natural and longer lasting result.

Every procedure has its tradeoffs, these tradeoffs will have to be weighed against your desired outcome and goals.
The tradeoff between this procedure and the traditional T- shaped scar (see the picture on the left), is two fold.
The circumareoalar technique will leave you with very limited incisions. This is excellent since we can avoid the other sometimes disfiguring scars. However, when we limit the incision to the areola, the closure of the wound is in a circle. As you can imagine, this closure will create wrinkles around the areola. In most patients this is not a problems since the wrinkling will improve tremendously over the next 2-3 months and it rarely requires a scar revision. However, if the lift required is just too much then the technique cannot be used. Exactly what is considered too much will be up to your Surgeons judgment, experience and philosophy.
The second drawback is that when the areola is closed in a circle it may flatten the areola. This is corrected to a certain degree when the implant is placed. This, therefore, is not a big issue.
