The Surgical Incision and Placement Choices.
Usually breast implants are surgically inserted utilizing one of four traditional incisions: the inframammary incision, the periareolar incision, the transaxilary incision, or the Umbilical incision.
Keep in mind that there is very little documentation to associate any particular incision with more or less risk of loss of nipple or other sensation. In fact, most doctors agree that loss of sensation has a lot more to do with implant size and surgical technique, both of which can dictate how large the area of insult to nerve endings will be inside the breast. Also, remember that there is no BEST incision. A very well healed incision in any location is better than a poor incision in any other location. What incision looks best depends on how it has healed and what position you are in, what you are wearing, at any given time. Your doctor should give you choices.
All of these incisions allow the implant to be placed either sub-glandular (between the chest muscle and your breast tissue) or sub-muscular (below the chest muscle).

The breast implant may be inserted directly under the breast tissue (sub-glandular) or beneath the chest wall muscle (sub-muscular).
No one incision and placement is right for every woman. Therefore, by knowing your options and discussing them with your doctor, together you can decide upon the best combination for you.
The inframammary incision is made in the skin fold below the breast, and the breast implant is inserted through this incision. The breast tissue is minimally disturbed, and the remaining incision scar is concealed within the fold of skin under the breast.
The periareolar incision is made along the edge of the colored area around the nipple, and the breast implant is inserted through this incision. The incision scar is well camouflaged in this area.
The transaxilary incision is made in the armpit and the implant is inserted through this incision.

An additional option that patients with ptotic or sagging breasts may elect to have during the breast implant surgery is a mastopexy (breast lift) with implants. A mastopexy (breast lift) can be done without implants as well.
